The Smoking Cessation program focuses on helping patients to quit smoking. A Registered Nurse provides you with support, counselling, and information on quitting smoking.

Stratford Family Health Team has partnered with the Centre for Addictions and Mental Health (CAMH) to offer the Smoking Treatment for Ontario Patients (STOP) Program. The STOP Program offers free Nicotine Replacement Therapy (NRT) in the form of patches, inhalers, gum, and lozenges, to eligible smokers (must be a resident of Ontario and a current smoker of cigarettes) who want to reduce their tobacco use or quit smoking.

Patient Status

You must be a patient of the Stratford FHT.

Referral

You do not need a physician referral to join this program.

Referral Criteria

You must be an Ontario resident and a current smoker of cigarettes.
