We are a Family Health Team operating out of two locations in Stratford, Ontario and Milverton, Ontario. Our team is comprised of family physicians, nurses, a nurse practitioner, physician assistants, a dietitian, an occupational therapist, mental health clinicians, and administrative support staff. Our multidisciplinary team utilizes their varied knowledge, skills, and experiences to deliver the very best care to our patients. We offer various programs, services, and groups to our patients that focus on education, health promotion, and chronic disease management.
