Regular Heart Health Services
A Registered Nurse assists patients in understanding the lifestyle factors that affect their heart health and how they can make changes to reduce their risk for cardiovascular disease. One on one visits with the registered nurse allow for patients to gain a better understanding of how they can make heart healthy choices through education, support, and follow up. Patients will have the opportunity to have their blood pressure assessed and cholesterol numbers reviewed. Additional referrals for diabetes, smoking cessation, exercise, stress management, and dietary support, are available as needed.
