Our Diabetes program provides education and support to patients with pre-diabetes and type 2 diabetes.
Service Provided:
- Individual and group education facilitated by a Registered Dietitian for persons with pre-diabetes and type 2 diabetes
- Education and support for management issues such as diet and exercise
- Referrals to Diabetes Education Center, other providers and programs at the SFHT as needed for management of medications, chronic complications, high blood pressure management/education, smoking cessation, and respiratory issues
