Our Diabetes program provides education and support to patients with pre-diabetes and type 2 diabetes. 

Service Provided:

  • Individual and group education facilitated by a Registered Dietitian for persons with pre-diabetes and type 2 diabetes
  • Education and support for management issues such as diet and exercise
  • Referrals to Diabetes Education Center, other providers and programs at the SFHT as needed for management of medications, chronic complications, high blood pressure management/education, smoking cessation, and respiratory issues

You must be a patient of the Stratford FHT.

You do not need a physician referral to join this program.
