Cardiac Rehabilitation is a 3-month education and group exercise program intended for those who have recently had a cardiac event or been diagnosed with heart disease and meet program eligibility requirements. Patients will learn about lifestyle changes they can make that will promote heart health, and set personal goals in collaboration with their care team. Referrals for diabetes, mental health support, smoking cessation, and dietary support, are available as needed and encouraged. The overall goal of the program is to promote a healthy lifestyle for improved heart health and reduce the risk of future cardiac events.
This program has a partnership with the YMCA in Stratford. Patients in the program will receive a free, 8-week membership to the Stratford YMCA in order to participate in the group exercise portion of the program.
