Our Team

Our Registered Nurses (RN) work closely alongside other healthcare practitioners to deliver patient care and run our various programs, groups, and services. Some of the programs and services our RNs run includes INR Clinic, Cardiac Rehab, and Wellness Clinic for Tots.

A Registered Dietitian (RD) is a regulated health professional who is highly knowledgeable about nutrition and food science. RDs promote health and help patients manage their chronic health issues through nutritional counselling. Nurse Practitioners (NP) are RNs with additional post-graduate university education and experience. NPs can autonomously perform physical exams, diagnose, prescribe medications, order medical tests, admit and discharge patients, provide referrals to other health practitioners, and more!

At the Stratford Family Health Team, we have registered mental health clinicians who provide short-term counselling to patients. Our mental health clinicians are experienced with different types of psychotherapy approaches and mental health issues. Occupational Therapists (OT) are regulated health professionals who provide support to patients with chronic conditions, illnesses, or injuries that are having difficulty living independently.
