The Memory Clinic provides memory testing and assessment for patients with memory loss to determine diagnosis, treatment, and future planning through a multi-disciplinary team approach. The Memory Clinic team consists of a Registered Nurse, Physician, and Social Service Worker. Patients will undergo various memory assessments that will aid in determining a diagnosis and treatment plan. Family and patients are connected with community resources to give home support as well as future planning if long term care is required.
