Mental Health – Collaborative Care

The Collaborative Care program offers comprehensive psychiatric assessments, medication consultations and short-term mental health treatment provided by an interdisciplinary team consisting of a psychiatrist and case managers.

Referral

You need a physician referral to join this program.

This program is for adults 18+.
