Through the Wellness Clinic for Tots program, a Registered Nurse will provide 18-month and 36-month developmental screening to eligible children using standardized recommended and evidence-based tools.

Through this program, 18-month and 36-month-old children will receive:

  • Up-to-date immunizations
  • Assessment regarding overall health, growth, and development
  • If needed, referrals to community resources to support child’s development

Possible topics covered in the visits include fine and gross motor skill development, speech and language, nutrition, safety, parenting strategies, toileting, and sleep.

Program Requirements

Patient Status

You must be a patient of the Stratford FHT.

Referral

You do not need a physician referral to join this program.

Referral Criteria

Child must be 18-months or 36-months of age.
