Pulmonary Rehabilitation

Pulmonary Rehabilitation is an 8-week education and group exercise program intended for those who have a chronic lung condition. Patients will learn about lifestyle changes they can make that will promote lung health and learn self-management techniques to manage their breathing symptoms to encourage the best possible quality of life. Referrals for diabetes, mental health support, smoking cessation, and dietary support, are available as needed and encouraged. The overall goal of the program is to promote self-management for improved lung health and reduce the risk of physician office visits and hospitalizations due to respiratory flares.

This program has a partnership with the YMCA in Stratford. Patients in the program will receive a free, 8 week membership to the Stratford YMCA in order to participate in the group exercise portion of the program. Home supported exercise is available as well if patients prefer.

Patient Status

You must be a patient of the Stratford FHT.

Referral

You do not need a physician referral to join this program.

Referral Criteria

This program is for patients with a diagnosed chronic respiratory condition (Asthma or COPD).
